Does anyone know why the option to rename a user would be grayed out when running SU01? I'm not sure if it's related to our CUA set up, but I haven't seen any notes or documentation on this anywhere.

I am not sure to fully understand your question but:
1. Enter the name of the user SU01 (DON'T PRESS ENTER)
2. Go the the menu ( User Names )
3. The rename will be available.
You can change the user name at the top level only. If you are visualizing the profile of the user you are to far.

This is greyed out in the CUA client. The only way is to copy the old username to the new one and then delete the old one.

Is it because CUA can't "handle" that type of request or is it something you can change to allow user admins to do by changing a setting somewhere. I'd really like to keep the audit trail of a rename.

Below is a response I got from SAP regarding this issue.
As the renaming functionality in the central system is quite complicated
and dangerous - it could produce easily inconsistencies between
the tables of the central and client systems - we have decided to
switch off the renaming functionality also in the central system.
