Would like to check with you is there any SAP standard tools/FM that can be used to extract all the user exits from the system according to Module? eg MM, PP, FI/CO, SD?
Can the result returned in "Include program name" instead of "Enhancement"?

By module, no, there isn't. There are other ways of finding UXs though, which are well documentated on sapfans & elsewhere.
