we have activated the appraisals, when a manager creates and appraisal for his subbordinated, it some time sends and email to the outlook and sometime to the sap inbox. Can anyone tell why it would be sending some time to sap inbox and sometimes to outlook. We want it to go to the outlook all the time..

it is hard to say without more information, you should check the recepient, SAP difer between external and internal adrress.
